Relay for Life at St. Mary’s HS is quickly approaching.  Our next step is to ensure that everyone who registered online to participate (sorry registration is now closed) has completed and submitted a Parent/Guardian Permission Form.

Click HERE to download a parent/guardian permission form, or go to room 221 (Student Success) to pick one up. Without a permission form, you will not be allowed to participate on Relay Day (May 30th).

The deadline to complete and submit forms is Friday, May 24th

Completed permission forms are to be dropped off in room 221 (Student Success).

NOTE:  if you are 18+ years old, you still must complete a permission slip in order to participate – you just get to sign it yourself.

3 steps to participate in Relay for Life at St. Mary’s

1) register (sorry, registration is now closed)
by May 13th

2) complete & submit a parent/guardian permission form
by May 24th

3) raise $50 or more in funds
(in addition to the $20 online registration fee)
by May 24th – but you can certainly continue fundraising until the event

top fundraising prizes – the top 3 fundraisers will be determined by 9 pm on May 29th
 – 1st prize – $100 gift card to Fairview Mall
 – 2nd prize – $50 gift card to Fairview Mall
 – 3rd prize – $25 gift card to Fairview Mall
