Relay for Life – Permission Form – Deadline – May 24
Relay for Life at St. Mary’s HS is quickly approaching. Our next step is to ensure that everyone who registered online to participate (sorry registration is now closed) has completed and submitted a Parent/Guardian Permission Form.
Click HERE to download a parent/guardian permission form, or go to room 221 (Student Success) to pick one up. Without a permission form, you will not be allowed to participate on Relay Day (May 30th).
The deadline to complete and submit forms is Friday, May 24th
Completed permission forms are to be dropped off in room 221 (Student Success).
NOTE: if you are 18+ years old, you still must complete a permission slip in order to participate – you just get to sign it yourself.

1) The deadline to register is this coming Monday, May 13th by 11:59 pm.
2) Moving forward, Relay for Life at St. Mary’s HS will be taking place every other year, which means our next event will be in the Spring of 2026. So, this year’s event will be the final opportunity for current grade 11 & 12 students to participate in RFL at St. Mary’s HS.
